Specialty Consultation
Consultative services include lifestyle medicine, travel medicine and executive health exams. These services can be schedule without establishing care as a new patient.
Primary Care Services
New primary care patients can schedule an optional discovery call at no cost. A new patient appointment is required to establish care. After establishing care, you have the choice to access service a la carte or through one of our membership options. In order to remain active as a current patient for primary care services, an annual preventive health visit is required every 12 months.
Labs & Imaging
Labs and imaging orders can be sent to your preferred lab or center that is covered by your insurance. If you do not have insurance we have arranged pre-negotiated cash prices for lab testing.
Pain Management
We do not offer chronic pain management or prescribe controlled substances such as opiates for pain management. A referral can be provided to a pain management specialist if needed.
Payment
We are an independent practice. We do not bill insurance or Medicare. Payment is collected at time of service. We accept cash, check, or credit card.
Cancellation Policy
If you need to reschedule or cancel an appointment we request that you provide at least 24-hours notice. Memberships can be canceled at any time with 30-days notice.
